Graduate Petition Form Instructions

To the Student Petitioner:

  1. State clearly and concisely the policy or regulation that your petition addresses. Petitions must be typed.
  2. State clearly and concisely the reasons you believe your petition should be approved; include a description of relevant extenuating circumstances.

  3. Attach all relevant documentation. This must include:
    • Transcript
    • Supporting statement from instructor and/or adviser.
    • Supporting statement from Departmental Graduate Committee Chair and/or Department Chair.

    If relevant, GO-11 or GO-12 form must be included. Lack of documentation will result in delay and return of the petition.

  4. Have the petition reviewed and signed by your faculty adviser.
  5. The completed petition must be presented to the Departmental Graduate Committee Chair and/or Department Chair.

To the Departmental Graduate Committee Chair and/or Department Chair:

  1. Verify that all relevant documentation has been supplied.
  2. Review the petition and make a recommendation for approval or denial.
  3. Attach supporting arguments for your recommendation.
  4. After appropriate Departmental Graduate Committee Chair and/or Department Chair signatures, forward the petition to the Office of Graduate Studies and Research. Deliver to the Unitus Building, 6th floor or mail to:

  5. Lack of documentation will result in delay and return of the petition
