Graduate Petition Form Instructions
To the Student Petitioner:
- State clearly and concisely the policy or regulation that your petition addresses. Petitions must be typed.
- State clearly and concisely the reasons you believe your petition should be approved; include a description of relevant extenuating circumstances.
- Attach all relevant documentation. This must include:
- Transcript
- Supporting statement from instructor and/or adviser.
- Supporting statement from Departmental Graduate Committee Chair and/or Department Chair.
If relevant, GO-11 or GO-12 form must be included. Lack of documentation will result in delay and return of the petition.
- Have the petition reviewed and signed by your faculty adviser.
- The completed petition must be presented to the Departmental Graduate Committee Chair and/or Department Chair.
To the Departmental Graduate Committee Chair and/or Department Chair:
- Verify that all relevant documentation has been supplied.
- Review the petition and make a recommendation for approval or denial.
- Attach supporting arguments for your recommendation.
- After appropriate Departmental Graduate Committee Chair and/or Department Chair signatures, forward the petition to the Office of Graduate Studies and Research. Deliver to the Unitus Building, 6th floor or mail to:
